The Valley Diaper Bank® (VDB)

The intake appointment involves a demographic form and needs assessment, and takes approximately 30 minutes. At intake, please bring the following:

 

  • The child who will be receiving diapers

  • The child's birth certificate

  • Current proof of residence (i.e. utility bills, rent or mortgage receipt, etc.)

  • Proof of current monthly gross income for all adult members of the household- copies are acceptable, all of the following are required

    • Two current bank statements showing direct deposit or other income deposits

    • One month of paystubs (four if paid weekly, two if paid bi-weekly or twice monthly)- must show gross pay and all deductions

    • If you are newly employed and have not yet received a paycheck, a letter from your employer on company letterhead stating your monthly gross, annual gross, or hourly wage and average hours worked is acceptable

    • Current cash assistance, food stamp, Social Security/Disability

    • Copy of your unemployment history - to print, visit the CT Dept of Labor website

 

The above information will be used to determine your eligibility for our program. After intake, income is collected six months later. Children are eligible for one year of service, or age 3 years- whichever comes first.
